CONFERENCE FEES

 

Full Conference Package

Includes admission to all conference sessions including the Meet & Greet Reception, Plenary, Concurrent and Poster Sessions, and breakfasts and lunches. There is an additional charge of $120.00 (plus tax) for the CAPHC 50th Anniversary Gala on Monday, October 22, 2018.

One-Day Session

Includes admission to all Conference sessions on the day of your choice, including breakfast and lunch. Sunday one-day registrations also include the Meet & Greet Reception.

All fees are in Canadian Dollars (CAD) and 5% GST will be applied.

* To qualify as a Student, applicants must submit a letter from your Institution as proof of full time enrollment as an under- or post-graduate student (excluding post-doctoral fellows). A photocopy of your Student ID card or letter can be submitted via email to conference@caphc.org. Students who do not provide proof of enrollment will be charged the Regular Full Conference rate.

Cancellation and Refund Policy / Cancellation Notifications

Please note that all Conference Registration fees, Social Event fees, and Accompanying Person fees, are subject to the Cancellation and Refund Conditions below and all fees are in Canadian dollars (CAD):

Cancellation notifications must be received in writing by September 15, 2018 – A refund will be granted less a $105.00 (plus tax) administrative charge per registration. No refunds will be issued after this date.

Cancellation notifications received after September 15, 2018 – 100% non-refundable.

Approved refunds will be processed and issued no later than November 17, 2018.

All cancellations and/or modifications must be submitted in writing to the CAPHC Conference Secretariat by email (conference@caphc.org).

 

Changes, Modifications or Name Substitutions

If you need to make any changes to your Registration, please contact us at conference@caphc.org.

If your modification requires assistance from the CAPHC Office, a $60.00 (plus tax) administrative fee will be charged.

An alternate attendee name may be substituted if you are unable to attend the CAPHC Conference. Substitutions are subject to a $60.00 (plus tax) administrative fee.

 

Re-Verification or Reprocessing of a Credit Card

Credit card payments that require reprocessing for any reason will incur a $60.00 (plus tax) administrative fee.
