Workshop Description

The Canadian Association of Paediatric Health Centres - Patient Safety Collaborative will be hosting a one-day Paediatric Patient Safety Workshop, entitled Advances in Paediatric Patient Safety in Canada.

This interactive workshop will focus on three ongoing CAPHC patient safety initiatives;

  • Key learnings from the experiences of the Paediatric Medication Reconciliation Collaborative will be explored, particularly in the context of the strategies for sustainability of the improvement in safer patient care realized by this QI initiative. The workshop will also provide an opportunity to learn about developing a business model for quality improvement initiatives.
  • The journey of the Child Health Program, WRHA in patient safety learnings achieved using the London Protocol to review critical incidents and near misses will be presented.  As part of this session participants will work in teams to review anonymized examples of critical incidents, identify contributory factors and develop potential solutions
  • In addition, results from CAPHC’s patient safety study entitled “Reducing Harm in Paediatric Care: Learning About Adverse Events and Near Misses Using a Validated Canadian Paediatric Trigger Tool” will be presented.

This promises to be an excellent networking and learning opportunity!
