CAPHC Patient Safety Collaborative – Paediatric Patient Safety Workshop
Advances in Paediatric Patient Safety in Canada
Winnipeg, Manitoba, March 31st, 2008
The Canadian Association of Paediatric Health Centres - Patient Safety Collaborative hosted a one-day Paediatric Patient Safety Workshop, entitled Advances in Paediatric Patient Safety in Canada. This interactive workshop focused on three ongoing CAPHC patient safety initiatives;
- Key learnings from the experiences of the Paediatric Medication Reconciliation Collaborative were explored, particularly in the context of the strategies for sustainability of the improvement in safer patient care realized by this QI initiative. The workshop also provided an opportunity to learn about developing a business model for quality improvement initiatives.

- The journey of the Child Health Program, WRHA in patient safety learnings achieved using the London Protocol to review critical incidents and near misses will be presented. As part of this session participants will work in teams to review anonymized examples of critical incidents, identify contributory factors and develop potential solutions
