CAPHC Patient Safety Sharing & Learning Tool

Patient safety is one of CAPHC’s national priorities. We are currently facilitating collaboration between members to address acute and vital issues in this area. Sharing of patient safety information between providers is one way to prevent similar adverse events from happening in different organizations. For that reason, the CAPHC Patient Safety Sharing and Learning Tool was developed to create a confidential and secure venue for sharing information about patient safety for children and youth. The following rationale guided work on the tool:

  • When a serious adverse event happens in one pediatric facility, it is in everyone’s best interest for the information to be rapidly disseminated to pediatric providers
  • In the absence of reporting/sharing of patient safety information between providers, it is more likely that additional adverse events of a similar nature will occur
  • Learning from the experiences of others creates great potential for promoting patient safety and for reducing patient morbidity and mortality

Purpose

The primary purpose of the Patient Safety Sharing and Learning Tool is to prevent the repeat of avoidable errors and adverse events, thus enhancing patient safety for children and youth. The Sharing and Learning Tool provides a confidential, secure, virtual pediatric collaborative for sharing patient safety information. Access to the » SHARING component of the tool (i.e. area for entry and submission of adverse event information) is limited to pre-authorized individuals who have been provided with the required login information (username and password) by CAPHC. These indidividuals include all CAPHC Liaisons, members of the CAPHC Patient Safety Collaborative, the Paediatric Chairs of Canada (PCC), and members of the CAPHC - Safer Healthcare Now! Paediatric Medication Reconciliation Collaborative (PMRC).

How the Tool works

The Sharing and Learning Tool is designed to support sharing of information about adverse events occurring at pediatric facilities and other related organizations, through voluntary reporting without identification of the facility, patient or providers involved. The Tool supports learning about the adverse events to prevent their recurrence. The Tool contains no identifying details, but includes enough information for providers to determine whether similar risks of adverse events exist in their own organizations. The » LEARNING component of the tool has been designed for general public access.

The Sharing and Learning Tool has been designed to be anonymous, however, if individuals are interested in discussing a particular event and/or related information within a more open forum, the

is available for this purpose.

How to use the Tool

  • To access the Tool and share patient safety information click on » SHARE.

    If you do not have pre-authorized access to share details of an adverse event but would like to report one, please contact the appropriate individual on the list of » Approved Adverse Event Reporters.
    If you are a member of the international community and and would like to report an adverse event please contact the CAPHC office.
