About KEN

The Knowledge Exchange Network (KEN) is the first national on line knowledge exchange resource that will ultimately provide one stop access to tools, templates, research, education and program information for health providers and families in issues related to improved continuity of care across different sites of care (i.e. hospitals, home, rehabilitation centres). The development of the KEN will be a multi-step and on-going process, allowing our communities of practice and families to share their knowledge and experience, as these evolve over time.

The initial component of the KEN will focus on the identification of regional practices related to transitions between centres and the community for children and youth with complex care needs. These practices will be identified on a voluntary basis by providers of health services across the country, and will be documented in a standardized template that encompasses a set of program and process considerations that lend themselves to a systematic review of programs. Ultimately, it is hoped that common themes will emerge around criteria and considerations with respect to leading practices, thus enabling the identification of potential child and youth health standards.

Leading Practices - Transition between Centres and the Community

As the area of transition services and supports is complex, and often tailored to a given local context, rigorous evaluation becomes prohibitive.  An experience-based and developmental approach has therefore been adopted, through the on-going identification, evaluation and sharing of practices and system improvements in order to achieve consensus in the future on leading practice characteristics.

Going Forward, CAPHC will work in collaboration with CCHSA and the hospital and continuing care sectors toward developing new standards (i.e. leading practices) across paediatric health care settings, ultimately resulting in significant improvements in family-centered health care for children and youth with complex care needs.
